POSITION PURPOSE:
Utilize person-centered, trauma-informed case management techniques to quickly assess the needs of people who are experiencing or at risk for experiencing homelessness, and then connect them with available resources to meet their emergent needs. Develop and maintain positive, collaborative working relationships with Together team members and external partners as needed to ensure quick service connections and positive outcomes for participants. Thoroughly and accurately document efforts in HMIS and/or other means as needed to meet funder requirements and assist Horizons leadership in measuring the effectiveness of various program approaches. Maintain flexibility in order to adapt to evolving program practices and priorities, assisting where needed to support the Horizons program and its participants.

 

ESSENTIAL JOB FUNCTIONS:
● Assist with management of the Together Horizons Crisis Engagement phone line, answering calls and
responding to voice mails as needed to address inquiries and requests for assistance in a timely manner.
● Over the phone and in person, facilitate housing problem-solving – interview people who have presented for
services in order to ascertain their needs; then assist them in developing and implementing plans to address
those needs.
● Facilitate referral to community resources, including but not limited to entities such as the Metro Area
Continuum of Care for the Homeless [MACCH] Housing Problem-solving Fund, Goodfellows, Dollar Energy,
St. Vincent de Paul, etc.
● Conduct MACCH Coordinated Entry assessments and make referral to the MACCH By-Name List (for HUDfunded housing programs targeting the population of people experiencing homelessness) when appropriate.
● Maintain current knowledge of community resources that may benefit people who are experiencing or at risk
for homelessness.
● Document all Crisis Engagement contacts and service plans in HMIS, Netsuite, and/or other data systems and
mechanisms as required by funders and outlined in program protocols.
● Maintain records in a professional, confidential, and orderly manner.

● Outstanding interpersonal, communication, planning, and organizational skills, with a positive, proactive
orientation to problem-solving and commitment to open dialogue
● Ability to work independently, effectively organizing time in a fast-paced environment and adapting to shifts
in priorities when needed, while also maintaining collaborative orientation
● Desire to learn and grow, with ability to embrace feedback and integrate new knowledge into approach
● Commitment to social justice and cultural proficiency in working with people from all backgrounds and family
compositions
● Engagement during assigned core hours, while maintaining flexibility to meet the scheduling needs of assigned
program participants
● Professional appearance and manner
● Proficiency in Microsoft Office applications, HMIS, and general office equipment
● Knowledge of community resources and service systems responsive to the needs of people experiencing or at
risk for homelessness

● Valid driver’s license and history of safe driving

 

EDUCATION AND EXPERIENCE
● Minimum of A.S. degree in Human Services or related field and/or one to three years’ experience working in
human services/social justice; prefer experience working directly with people experiencing or at risk for
homelessness.
